Comprehensive
Case Management Software
MMARS utilizes a fully integrated and custom system to provide
your population with quality case management, and to provide you with
the best information possible.

Integrated
assessment reports. Our system has a number of
formats for assessment reports, allowing us to provide you with the
assessment documentation that you require. From short one or two
page assessments, to fully comprehensive assessments spanning ten
pages or more, we can provide the level of detail that you need. If
you have specific needs that our existing formats do not meet, we
can work with you to develop custom formats to capture the
information that you need.

Integrated
assessment and information gathering. Descriptive reports
help you to document individual cases, but are not always conducive
to data analysis across your entire care population. Our system
integrates data collection with coded responses, numeric and other
data formats, in order to provide you with the data analysis that
you need. Outcomes information, population statistics, outliers and
other data help you to understand your population and for us to
better serve you. We work with our clients to help define the data
set that you need to have gathered. We can provide different data
sets for subpopulations if required also.

Integrated
care plans. Unlike other systems, ours does not simply
provide a suggested careplan to our case managers based upon a
handful of templates that are diagnosis based. Our careplan system
integrates with our assessment tools, and helps to build a careplan
based upon the information gathered during an assessment. If an
individual is non-compliant in a certain area, the system will add
this into the careplan during the assessment process. In turn,
issues and goals within the careplan can trigger follow-up questions
during future assessments.

Activity
tracking and documentation. All activities performed by our
case managers are tracked, coded and documented within our system.
This allows us to provide you with the detail that you require, but
also allows us to ensure the highest level of case management
possible. Our system allows us to track activity indicators to
verify contract compliance. These can be count based (1 visit a
month), time based (3 hrs of visit time per month), report based
(minimum of 1 visit and no more then 2 visits per assessment report)
as well as trigger based. We have an incredible degree of
flexibility to make certain that our case managers are performing
the tasks that you require.

Cost
savings tracking. Our system tracks and codes cost savings.
It allows for analysis of savings in relation to a number of
variables. Most importantly, it distinguishes between hard and soft
savings so that savings reports provide accurate (non-inflated)
figures to you.

Event
tracking and prompting. Case managers are prompted
when specific events are due, such as completing reports. Event
completion is also tracked.
The
system also provides numerous other functions to help our case managers
service your population better, for us to manage our case managers
better, and for us to provide you with the information that you need.
