MMARS Disease Management is holistic in approach and integrated with case management services. To decrease fragmentation of care and improve outcomes of care MMARS cross trains case managers to provide both disease management and case management depending on the intensity of need.

MMARS Disease Management Tools

• Are flexible to include national guidelines for care, clinical guidelines developed by the provider organization / PCP and other contract specific guidelines 
• Guide the case manager in quality disease-based care 
• Are used to collect outcome information that can help determine the impact of disease management on utilization patterns, health status and improved health-seeking behavior. 
• Trigger issues to the case management plan of care so that the problem areas identified are the focus of the case management interventions
